The problem
An assessment is commissioned to unlock something: a permit, a disbursement, a board decision. That creates quiet pressure for the finding to arrive at the answer somebody already needs, and everyone reading it knows that.
The second failure is quieter. The study is sound, the project goes ahead, and nothing is ever measured again — so nobody can say afterwards whether the impact materialised, and the next proposal starts from assertion all over again.
